Practices on logging the demand on the shutdown valves in a safety event?

One of my colleagues in the Middle East passed along concerns he's heard about industry practices on logging the safety event in case of "Demand" for a final control element.

He's seen older plants have limit switches mounted on shutdown valves, which log the valve travel in the HOST system. Due to reliability issues they prefer to use position transmitter mounted on the shutdown valves as an independent means to log the valve travel in the HOST system.

What is your practice with respect to logging the demand on the shutdown valves in a safety event?
